This is the appointment that quietly prevents most of the expensive ones. Plaque that is not removed within a couple of days hardens into tartar, and once it has hardened no amount of brushing will remove it — it has to be taken off with instruments.

A cleaning on gums that have been maintained is a shorter piece of work than a first cleaning after several years away. If cleaning below the gumline turns out to be needed, that is gum treatment rather than a routine clean, and we will tell you the figure before starting rather than after.
What happens during a cleaning
- Examination first. We check the teeth, gums and soft tissues, and look at how much build-up is present and where.
- Scaling. Hardened tartar is removed from the tooth surfaces and from just under the gumline, where it does the most damage.
- Polishing. This lifts surface staining and leaves the teeth smooth, which slows down how quickly new plaque sticks.
- Advice specific to you. Where you are missing when you brush, and what would make the biggest difference.
If your gums are inflamed the cleaning may feel more sensitive — that inflammation is exactly the reason to have it done.
Signs you are overdue
Most people book a cleaning by the calendar. These are the signs that say do not wait for it:
- Gums that bleed when you brush or floss. Not normal, and the earliest reliable signal. Why gums bleed.
- A rough or gritty feel behind the lower front teeth — usually the first place tartar builds.
- Persistent bad breath that brushing does not fix. What actually causes it.
- Gums that look red or puffy rather than firm and pale pink.
- Teeth that look longer, or a dark line appearing at the gumline.
- It has been more than a year. Tartar does not come off on its own and it does not stop accumulating.
None of those mean anything is seriously wrong. They mean the cleaning is due, and that having it now is a smaller job than having it later.
Does it hurt?
A routine cleaning on healthy gums is not painful. It is noisy, there is water, and some people find the vibration odd — but it does not hurt.
If your gums are inflamed it can be tender, and that tenderness is the inflammation rather than the instrument. It settles within a day or two, and it is the reason to have the cleaning rather than a reason to avoid it.
A deep clean is a different appointment. If tartar has built up below the gumline, cleaning the root surface is gum treatment, not a routine clean — the area is numbed first, so it is comfortable, and it is usually done over more than one visit so each side is treated properly rather than hurried. Will it whiten my teeth?
Partly, and it is worth being precise about the difference because people are often disappointed by the answer they were not given.
A cleaning removes surface staining — the tea, coffee, red wine and tobacco that sits on the enamel. For a lot of people that alone makes teeth look noticeably brighter, because what they were seeing was stain rather than tooth colour.
What it does not change is the natural colour of the tooth underneath. If that is what you want changed, it is whitening, which is a different treatment — and the sensible order is cleaning first, because whitening works better on a clean surface and you may find you need less of it than you thought.
Why brushing alone is not enough
Brushing cleans the surfaces your brush can physically reach. It does not reach between the teeth, which is roughly 40% of each tooth's surface — the area where cavities and gum disease most often begin. That is the case for interdental cleaning, and it is why we talk about flossing so much.
But even excellent brushing and flossing cannot remove tartar once it has hardened. That part requires instruments, which is what this appointment is for.
