Nightguards
Short answer: A nightguard is a custom-made cover, moulded from impressions of your own teeth, worn during sleep so that grinding — clinically, bruxism — wears the guard instead of your teeth. It does not stop grinding — it makes it harmless. A custom guard fits precisely enough to be worn all night, which is the main reason it works where a pharmacy guard does not.
Why the pharmacy version disappoints
Boil-and-bite guards are cheap and genuinely better than nothing for a short period. They also tend to end up in a drawer, for reasons worth knowing before you spend the money:
- They are bulky, so they are uncomfortable enough that people take them out in their sleep.
- They fit approximately. A guard that does not seat precisely can let teeth move slightly over months.
- The material is softer, and for a heavy grinder a soft guard can actually encourage more chewing activity.
- They wear through quickly.

What is involved

Most people take a few nights to get used to it. Bring it to routine appointments so we can check both the guard and whether the wear on your teeth has stopped progressing — that is how we know it is working.
Do you actually need one?
If your teeth show grinding wear, a guard is preventive rather than optional — rebuilding a worn-down bite later is among the most expensive work in dentistry, and a guard costs a small fraction of it.
If you are unsure, that is exactly what an examination answers. And if someone has proposed extensive crown work to "fix" grinding damage, that is a reasonable moment to get a second opinion first.

What moves the figure: which appliance the assessment says you need, the material and thickness for how hard you grind, and whether the guard goes on the upper or lower teeth.

Grinding and clenching — bruxism, clinically — does not only wear teeth. It loads the muscles and the joint, and the commonest thing people notice is not the wear at all: it is jaw pain, a dull ache at the side of the face, or a headache that is worst on waking.
A nightguard protects the teeth from the force. It does not, on its own, treat a joint that has become painful — and if your jaw clicks, locks, or aches through the day rather than just in the morning, that needs assessing rather than covering up. Say which one you have when you come in. A guard for tooth wear and a plan for a painful joint are different things, and the assessment sorts one from the other.
